The Pharmacotherapy Team provide a range of services through pharmacist, pharmacy technician and pharmacy assistant support, including:
- Acute prescribing (non-regular medication)
- Repeat prescribing (regular medication)
- Serial prescribing (setting-up regular medication through Community Pharmacies)
- Medication and polypharmacy reviews
- Medicines reconciliation after hospital discharge or clinic letter
- Medication enquiries
- Medicine safety reviews/recalls
- Monitoring high risk medicines
- Non-clinical medication reviews
- Specialist clinics (e.g. diabetes, etc.)
