- Forres Neighbourhood Care Team (FNCT) and District Nursing Service
We understand that people prefer to remain in their own homes during times of poor health and this can often improve their recovery. The Forres Community Nursing Teams can help housebound patients do this by providing support and care in situations of social and clinical need. The Community Nursing Team are skilled Nurses and Healthcare Support Workers who can provide an assessment of health and social care needs in the patient’s own home and they can also refer on to other services such as physiotherapy and occupational therapy or arrange prescriptions or medication reviews. We can provide:- Personalise healthcare assessments
- Onward referral to other services
- Long-term condition management, education and self-management support
- Wound assessment and treatment
- Continence assessment and support
- Palliative and end-of-life care and family support
- Anticipatory care / rehabilitation
- Support and education of family and/or formal carers
- Complex or acute care at home
- Health promotion and lifestyle advice
- These teams are based at the Health Centre. They work 7 days per week, providing Nursing cover from 8am-6pm, and Healthcare Support Worker cover from 8am-8pm. The team can be contacted by calling 01309 678899.
