Medication Reviews
Who do I see?
Advice:
The doctors and pharmacotherapy team at the Practices regularly review your regular medications on your Repeat Prescription. This is to ensure that the medicines you are taking are still appropriate for your needs and also to check that they are still safe. This process may involve making changes to your tablets, for a blood test to be arranged or for the need to speak with you.
If we need to speak with you to review your medication you may receive a telephone message, a text message or a letter from the practice to arrange a medication review appointment. Please follow the specific advice in the message that you receive. Please call us on 0345 337 1120 to book an appointment.
If you would like to discuss any issues with the medication you are currently taking please submit an online medical consultation request for the GP Clinical Team giving us as much information about your issue as you can. We will respond usually with an offer of an appointment with a GP or other suitable clinician to assess your problem further. We may also ask our pharmacotherapy team to deal with the issue if is more straightforward.
You may also consider speaking to the pharmacotherapy team directly on the medication queries line (available on 0345 337 1120, option 3, between 10 AM and 4 PM) if you think the query is more straightforward or is a technical query such as relating to the number of tablets supplied, etc., they can always advise you if you need to speak to a clinician.
For any more urgent queries or if you think you are having a bad side effect or reaction to one of your medicines, please submit an online medical consultation request for the Urgent Medical Problem Team giving us as much information about your problem as you can. You will get a same-day response usually by telephone or a time to attend the surgery for further assessment and examination.
If you cannot use the online service please call us on 0345 337 1120 and a member of the care navigation team will help you.
