If you have been asked to send a clinical photo or document, please use the upload form below.

For any documents in a foreign language, for example, a scan result or a care/discharge summary if you were seen as an emergency abroad; it is your responsibility to have these translated into English before uploading them (your travel insurance company may be able to help with this).

This will then be sent to our secure NHS mailbox and any photos or documents added to your clinical record. Please do not upload documents or images unless you have spoken to our team first.  

Your clinician may ask you to send photographs to assist with diagnosis. As such, these images will then become part of your medical record and will be treated with the same standards of confidentiality as any other part of your medical record. The image(s) will be reviewed by the requesting clinician and may be shared with other members of the practice team or forwarded to other healthcare professionals for advice. Any member of the practice team who may have access to such images is bound by strict confidentiality standards. We will normally contact you within the next 24 to 48 hours, once your information has been reviewed.

Here is our 10-step guide for taking the most helpful photos for our clinicians
  1. Use the best quality phone (or digital) camera you have.
  2. When taking photos take several then pick the best ones.
  3. Make sure the skin area is well-lit.
  4. Try to always orientate the photos such that person’s head is at the top of the image.
  5. Take a photo from a distance so that the lesion or rash has context (e.g. the whole of person’s back) then a mid-close-up with an anatomical marker (e.g. navel).
  6. For close-up photos of skin lesions (e.g., a mole) try to take these from 20 cm away from the skin. Take two images from different angles keeping the lesion in the centre of the image.
  7. Place a ruler, tape or coin next to the lesion (if possible) so we have a sense of its size.
  8. If possible, place a solid neutral colour (e.g., a towel) in the background.
  9. Disregard any poor images (e.g., out of focus), sending us only the best ones.
  10. You can send it as a JPEG or whatever format your phone/camera uses.
